At Care4uWE, we conduct thorough assessments with each new referral, gathering essential information about the individuals involved. We utilize our 'Assessment and Risk Decision-Making Tool' to evaluate this information comprehensively.
With the help of this tool it enables us to fully understand individual needs, ensuring a smooth transition into semi-independent living with the best possible support.
All placements are made in accordance with the Care4uWE referral, admission, and discharge policy. Our goal is to deliver person-centered assistance tailored to the individual’s risk assessment, care/support requirements, and budget considerations.

Assessment
The assessment process is structured to ensure we can offer placements where individuals can attain the most favorable outcomes. We assess the whole person to evaluate their fit for specific homes or services, taking into account their needs, abilities, preferences for living arrangements, social connections, and what is most significant in their lives. Our aim is to support them in achieving their fullest potential while ensuring a safe, happy, and fulfilling lifestyle.
After the initial assessment, we encourage the individual being referred, along with their family, friends, and care manager, to visit potential accommodations to meet our current residents and staff members. If visits are impractical or unsuitable, we seek insights from someone familiar with the individual to provide recommendations.
Once all involved, including the individual in need of support, agree that a placement is suitable, their care manager will initiate the funding application. Upon securing funding, we will confirm a start date and create a personalized transition plan to provide optimal support during and after the relocation process.
